ALLERGY

OBJECTIF: Évaluer l'innocuité des inhibiteurs de la COX-2 (coxibs) chez les patients asthmatiques avec une hypersensibilité à l'aspirine.

SOURCE DES DONNÉES: Les études cliniques oubliées en langue anglaise ont été identifiées à l'aide de MEDLINE (1966 à septembre 2002) avec les mots-clés suivants: inhibiteurs de cyclo-oxygénase, aspirine, asthme, et hypersensibilité. Les références des articles identifiés ont également été vérifiées.

SYNTHÈSE DES DONNÉES: La littérature contient des informations sur l'innocuité des coxibs chez les patients asthmatiques avec une composante respiratoire exacerbée par l'aspirine. Cette dernière est causée par l'inhibition de la cyclo-oxygénase, particulièrement la COX-1. L'inhibition de la COX-1 entraîne une production accrue de certains médiateurs inflammatoires qui résultent en des réactions reliées aux maladies respiratoires exacerbées par l'aspirine. Considérant ce mécanisme, les coxibs pourraient constituer une alternative à l'aspirine ou aux anti-inflammatoires non stéroïdiens (AINS) chez ce type de patients. Cet article analyse 4 études évaluant l'innocuité des coxibs chez cette population.

RÉSULTATS: Les 4 études ont évalué un total de 172 patients. Tous les patients démontraient une intolérance à l'aspirine ou aux AINS mais ont toléré l'administration de coxibs.

CONCLUSIONS: Les coxibs constituent une alternative sécuritaire pour le traitement de conditions inflammatoires chez les patients atteints de maladie respiratoire exacerbée par l'aspirine.
