OBJECTIF: Rapporter une interaction entre une préparation multivitaminique contenant 25 µg de vitamine K1 et la warfarine dans une série de cas et évaluer la prévalence d'une déficience en vitamine K1 chez des patients ambulatoires anticoagulés.

RÉSUMÉ: L'histoire de 3 patients stabilisés à la warfarine ayant soit initié ou cessé la prise d'une préparation multivitaminique contenant 25 µg de vitamine K1 laquelle fut associée à une chute ou une élévation inexpliquée de leur RNI en présence de thrombose majeure ou d'hémorragie chez 2 patients respectivement est rapportée. Cette interaction fut catégorisée probable selon l'échelle de causalité de Naranjo. Suspectant une déficience en vitamine K1 comme responsable de cette sensibilité accrue, la prévalence de déficience en vitamine K1 auprès de 179 patients consécutifs de la clinique fut déterminée par la mesure plasmatique de vitamine K1. Des niveaux plasmatiques très bas (<0.1 ng/mL) furent obtenus chez 12% patients.

DISCUSSION: Un supplément vitaminique contenant 25 µg de vitamine K1/j est inférieur à la dose connue comme affectant le contrôle anticoagulant. Il est possible que certains patients aient une déficience en vitamine K1 et que celle-ci ait causé une hypersensibilité à un supplément vitaminique contenant de petites doses de vitamine K1. Chez les patients ayant une telle déficience, même de petites doses représentent une augmentation significative dans leur apport diminuant ainsi la sensibilité à la warfarine. L'analyse des taux plasmatiques de vitamine K1 révèle qu'il existe une minorité de patients anticoagulés ayant un faible taux de vitamine K1.

CONCLUSIONS: Les cliniciens devraient questionner leurs patients anticoagulés sur la prise de suppléments vitaminiques et surtout suspecter une telle prise lors de changements inexpliqués du RNI.
