OBJECTIF: Rapporter l'efficacité et l'innocuité d'une thérapie chronique à faible dose de cyclophosphamide chez un enfant ayant une hémosidérose pulmonaire idiopathique (HPI).

RESUME DU CAS: Un garçon de 7 ans diagnostiqué avec une HPI il y a 4 ans, fut initialement traité avec la prednisolone. À cause d'une réponse partielle au stéroïde, la cyclophosphamide 2 mg/kg/j fut ajoutée. Une amélioration majeure fut observée lors des rendez-vous subséquents. Un an plus tard, il a développé une thrombocytopénie (75 x 103/mm3) et ce taux a chuté jusqu'à 10 x 103/mm3 durant les 10 mois suivants. La dose de cyclophosphamide fut diminuée à 1 mg/kg aux 2 jours et le décompte plaquettaire augmenta entre 20 et 50 x 103/mm3 sans le développement de pétéchies ni de saignement spontané. L'HPI est demeurée en rémission durant l'année qui s'ensuivit.

DISCUSSION: Due à la faible prévalence de l'HPI, très peu de données existent sur l'efficacité et l'innocuité des thérapies immunosuppressives. Bien qu'une réponse bénéfique soit survenue avec la cyclophosphamide à faible dose, le patient a développé une thrombocytopénie. Le mécanisme n'est pas élucidé mais il peut être le même que celui causant la myélosuppression induite par de hautes doses de cyclophosphamide. À cause du développement de thrombocytopénie, la cyclophosphamide fut maintenue à dose réduite. Les bénéfices d'une telle thérapie à long terme sont controversés et une plus grande expérience est requise pour supporter son utilisation.

CONCLUSIONS: Une thérapie chronique à dose réduite de cyclophosphamide est efficace pour traiter l'HPI chez l'enfant mais une attention particulière devrait être portée au décompte plaquettaire. Un monitorage périodique des plaquettes est recommandé.
