OBJECTIF: Évaluer l'efficacité et l'innocuité de la tinzaparine pour la prévention et le traitement de la thromboembolie veineuse.

MÉTHODOLOGIE: Recherche informatisée dans la banque de données MEDLINE et PubMed d'articles de langue anglaise publiés entre 1980 et décembre 2002. Les articles discutant de l'innocuité, de l'efficacité, et du rapport coût-efficacité de la tinzaparine ont été retenus et évalués. Une emphase particulière a été placée sur le choix d'études à répartition aléatoire avec témoin.

RÉSULTATS: La tinzaparine est une héparine de faible poids moléculaire qui exerce son activité anticoagulante principalement par une inhibition des facteurs de coagulation Xa et IIa. La tinzaparine est indiquée pour le traitement de la thrombose veineuse profonde et de l'embolie pulmonaire ainsi que pour la prévention de la thromboembolie veineuse. L'administration quotidienne de la tinzaparine par voie sous-cutanée est aussi efficace que l'héparine intraveineuse non fractionnée pour la prévention et le traitement de la thromboembolie veineuse et elle est certes tout aussi sécuritaire d'utilisation quant à son potentiel d'effets secondaires de type hémorragique. Aucun ajustement posologique n'est requis pour les patients obèses, les patients âgés, et les insuffisants rénaux bien qu'un monitoring soit recommandé pour ces 2 dernières populations. Des données comparatives préliminaires suggèrent que l'efficacité de la tinzaparine est tout à fait comparable aux autres héparines de faible poids moléculaire disponibles sur le marché. La publication d'études pharmacoéconomiques indique aussi un rapport coût-efficacité favorable.

CONCLUSIONS: La tinzaparine possède une efficacité démontrée pour la prévention et le traitement de la thromboembolie veineuse et un profil d'innocuité favorable. L'utilisation de la tinzaparine dans le milieu ambulatoire peut être avantageux due à son administration quotidienne par voie sous-cutanée.
