OBJECTIF: Revoir les théories biochimiques actuelles concernant la contribution du diabète aux lésions microvasculaires.

REVUE DE LITTÉRATURE: Une recherche informatisée MEDLINE couvrant la période de 1980 à aujourd'hui ainsi qu'une recherche à l'aide de bibliographies d'articles obtenus furent effectuées.

SÉLECTION DES ÉTUDES ET DE L'INFORMATION: Les articles identifiés furent tous évalués.

RÉSUMÉ: Les théories biochimiques actuelles concernant la contribution du diabète aux lésions microvasculaires incluent l'augmentation de la voie des polyols (sorbitol/aldose réductase), la production de produits glyqués, la production d'espèces réactives oxygénées, et finalement l'activation du diacylglycerol et de la protéine kinase C. Toutes ces voies biochimiques contribuent aux dommages et à la dysfonction microvasculaires et peuvent altérer le fonctionnement génomique.

CONCLUSIONS: Les mécanismes de ces diverses voies biochimiques se chevauchent et contribuent aux altérations de la fonction microvasculaire responsable du développement de la rétinopathie, neuropathie, et néphropathie, les principales complications microvasculaires du diabète.
