OBJECTIF: Rapporter un cas d'interaction médicamenteuse entre la warfarine et l'amoxicilline/clavulanate de potassium qui a entraîné une élévation du ratio international normalisé (RIN) et de l'hématurie.

RÉSUMÉ DU CAS: Une femme de 58 ans a présenté un RIN élevé ainsi que de l'hématurie macroscopique résultant d'une interaction médicamenteuse entre la warfarine et l'amoxicilline/clavulanate de potassium.

DISCUSSION: Le cas rapporté dans le présent article traitant d'une augmentation du RIN avec complications suite à une interaction médicamenteuse entre la warfarine et l'amoxicilline/clavulanate de potassium est comparable à ceux que l'on retrouve dans la documentation scientifique. Le mécanisme de cette interaction n'est pas complètement connu. L'hypothèse la plus plausible mentionne qu'il pourrait s'agir d'une diminution de la flore intestinale productrice de vitamine K, entraînant ainsi une déficience en vitamine K. Selon l'algorithme de Naranjo, la relation entre l'effet indésirable observé et l'administration concomitante de warfarine et d'amoxicilline/clavulanate de potassium est possible.

CONCLUSION: L'augmentation du RIN résultant d'une interaction entre la warfarine et un agent antimicrobien est un phénomène connu. Compte tenu de l'interaction possible, l'administration combinée de warfarine et d'amoxicilline/clavulanate de potassium nécessite un monitoring approprié afin de contrôler le RIN et de prévenir les complications hémorragiques.
