OBJECTIF: Rapporter le cas d'une cellulite et d'une bactériémie à Pasteurella multocida traitées avec succès à l'aztréonam.

RÉSUMÉ DU CAS: Un vieil homme de 81 ans connu pour allergies à de multiples antibiotiques est admis pour une cellulite sévère au bras gauche et une bactériémie causées par le P. multocida. Le patient a été traité durant 14 jours avec de l'aztréonam et l'infection est complètement disparue.

DISCUSSION: D'après nos connaissances, ce cas représente le premier cas décrit publié de traitement d'une cellulite et d'une bactériémie ayant répondu avec succès à l'aztréonam. Les antibiotiques recommandés pour le traitement des infections à P. multocida incluent les pénicillines, l'ampicilline, l'amoxicilline, les céphalosporines de deuxième et de troisième génération, les tétracyclines, les fluoroquinolones, et le triméthoprim/sulfaméthoxazole. Il existe peu de données dans la littérature actuelle concernant l'activité de l'aztréonam envers le P. multocida.

CONCLUSION: Ce cas illustre l'utilisation potentielle de l'aztréonam lors de cellulite et de bactériémie causées par le P. multocida lorsque les autres antibiotiques de choix ne peuvent pas être employés.
