OBJECTIF: Présenter un cas de rhabdomyolyse non traumatique compliquée d'insuffisance rénale aiguë oligurique, à la suite d'un surdosage intentionnel d'éthanol et de diphénhydramine.

RÉSUMÉ DU CAS: Un homme blanc de 21 ans a été reçu aux urgences à la suite d'un surdosage intentionnel d'éthanol et de diphénhydramine. Le patient a développé par la suite une insuffisance rénale aiguë et le diagnostic de rhabdomyolyse non traumatique a été posé. Etant donnée l'absence d'autre cause commune dans ce cas, la rhabdomyolyse a été attribuée au surdosage de l'éthanol associé à la diphénhydramine.

DISCUSSION: La rhabdomyolyse est un syndrome sévère potentiellement mortel causé par des atteintes du muscle squelettique, y compris des atteintes induites par des médicaments. La détection précoce et l'instauration de traitements efficaces sont essentiels pour minimiser les complications de ce syndrome. Un retard de diagnostic a largement contribué dans ce cas à la sévérité de l'insuffisance rénale.

CONCLUSIONS: La rhabdomyolyse est un événement indésirable rare résultant de l'ingestion de toxines et de médicaments. Le médecin doit conserver un haut degré de suspicion devant des cas d'ingestion associée à des troubles musculo-squelettiques afin de permettre un diagnostic et un traitement précoces.
