OBJECTIF: Rapporter un cas de purpura thrombocytopénique autoimmun (PTA) survenu dans le contexte d'une leucémie lymphocytaire chronique (LLC) et traité avec la fludarabine.

RÉSUMÉ: Une femme âgée de 68 ans, souffrant de LLC, a été traitée avec du chlorambucil oral. Par la suite, elle a présenté un PTA grave. La gammaglobuline intraveineuse fut administrée sans succès et une splénectomie thérapeutique a été pratiquée, ce qui a permis une augmentation temporaire du décompte des plaquettes (65 x 103/mm3). Quatre semaines plus tard, le décompte des plaquettes était à nouveau abaissé à moins de 10 x 103/mm3. A ce moment, la patiente fut traitée avec de la fludarabine. Le décompte des plaquettes effectué 4 semaines plus tard était normal (270 x 103/mm3) et est demeuré normal pendant plus de 8 mois.

DISCUSSION: La fludarabine a été associée à l'apparition de cytopénies autoimmunes lors de LLC. Ce cas suggère que la fludarabine pourrait constituer un traitement potentiel pour les cas de PTA associés à la LLC et réfractaires au traitement usuel.

CONCLUSIONS: La fludarabine pourrait être utile en traitement du PTA survenant dans la LLC et réfractaire au traitement conventionnel.
