OBJECTIF: Revoir la littérature comparant l'efficacité du calcium, de l'hormonothérapie de remplacement, des biphosphonates, des modulateurs sélectifs des récepteurs oestrogéniques, et de la calcitonine dans la prévention et le traitement de l'ostéoporose post ménopausique.

SOURCE DES DONNÉES: Les articles ont été identifiés dans les banques de données MEDLINE, EMBASE, et International Pharmaceutical Abstracts en utilisant les mots clés ostéoporose, fracture, calcium, vitamine D, hormonothérapie de remplacement, biphosphonates, alendronate, risédronate, raloxifène, et calcitonine. Des références supplémentaires ont été identifiées en utilisant les bibliographies des articles cités.

SÉLECTIONS DES ÉTUDES ET EXTRACTION DES DONNÉES: Les études expérimentales et d'observation sur l'utilisation du calcium et des agents antirésorptifs pour la prévention et le traitement de l'ostéoporose post ménopausique ont été sélectionnées. Les articles traitant de la densité minérale osseuse ou l'efficacité au niveau des fractures ont été inclus dans cette revue.

ANALYSE DES DONNÉES: L'hormonothérapie de remplacement, les biphosphonates, le raloxifène, et la calcitonine ont tous été démontré une amélioration dans la stabilisation et l'amélioration de la densité minérale osseuse. Les études randomisées ont démontré une diminution du risque de fracture avec les biphosphonates, le raloxifène, l'hormonothérapie de remplacement, le calcium, et la calcitonine; la plus grande réduction ayant été observé avec les biphosphonates.

CONCLUSIONS: Différentes options thérapeutiques avec des améliorations bien documentées de la densité minérale osseuse et la réduction du risque de fracture sont disponibles pour les femmes dans la prévention et le traitement de l'ostéoporose post ménopausique.
