OBJECTIF: Faire le point sur les informations actuellement disponibles sur la rasburicase dans le traitement et la prévention de l'hyperuricémie.

SOURCES DE DONNÉES: Recherche de publications et de revues d'articles dans MEDLINE (1996–août 2002).

SÉLECTION DES ÉTUDES/EXTRACTION DES DONNÉES: Les études d'évaluation de la rasburicase, ainsi que les résumés et comptes-rendus de communications ont été considérées pour la sélection. La littérature en anglais a été évaluée sur le plan de la pharmacologie, la pharmacodynamie, la pharmacocinétique, l'usage thérapeutique, et des effets indésirables de la rasburicase.

SYNTHÈSE DES DONNÉES: La rasburicase, urate-oxydase recombinante, a fait preuve de son efficacité dans l'abaissement de l'acide urique et la prévention de son accumulation chez les patients atteints d'hémopathies malignes, avec hyperuricémie ou à risque élevé de développer une hyperuricémie. Elle a été approuvée pour l'utilisation en pédiatrie aux USA.

CONCLUSIONS: Avec l'allopurinol, l'hydratation, et l'alcalinisation urinaire, la rasburicase représente une nouvelle alternative dans le traitement et la prévention de l'hyperuricémie chez les patients atteints d'hémopathies malignes. Sa rapidité d'action et sa capacité à abaisser les hyperuricémies préexistantes constituent les avantages de la rasburicase par rapport à l'allopurinol. Elle peut permettre au patient de bénéficier d'une chimiothérapie sans le moindre délai.
