OBJECTIF: Décrire le processus de développement de recommandations pour l'utilisation initiale des héparines à bas poids moléculaires (HBPM) et autres anticoagulants dans des hôpitaux de soins aigus faisant partie d'un groupe d'achat national (GAN).

REVUE DE LITTÉRATURE: Une recherche systématique de la littérature fut effectuée pour identifier l'évidence supportant l'efficacité de divers anticoagulants dans le traitement initial de thrombose veineuse profonde et de l'embolie pulmonaire et dans le traitement du syndrome coronarien. La méthode par consensus fut utilisée pour développer ces recommandations. Celles-ci furent revisées par un panel interne et externe. Une version finale des recommandations fut disséminée à tous les membres du GAN et une aide fut offerte pour leur implantation locale.

RÉSULTATS: Les recommandations finales sont décrites. Elles sont organisées en fonction des options thérapeutiques pour chaque indication. Pour chaque option, l'opinion du panel est fourni sur le niveau d'évidence qui existe dans la littérature, sur une comparaison des coûts et sur toute information additionnelle relative à la posologie. Ces recommandations furent disséminées aux hôpitaux membres du GAN intéressés et une téléconférence a eu lieu pour faciliter l'implantation locale des recommandations. Celles-ci furent implantées à travers le pays.

CONCLUSIONS: Les recommandations finales et le processus de développement de ces dernières peuvent être utiles à d'autres hôpitaux/organisations qui envisagent une revue de ces médicaments en vue d'une sélection.
