HEMATOLOGY

OBJECTIF: Promouvoir la prophylaxie des thromboses veineuses profondes selon les recommandations nationales tout en minimisant les coûts associés à la médication.

MÉTHODOLOGIE: Les patients admis à notre institution et ayant reçu une thromboprophylaxie utilisant une héparine à bas poids moléculaire (HBPM) telle l'énoxaparine furent identifiés et évalués pour une conversion à l'héparine non fractionnée à faible dose. Une population de patients admis sur une unité de médecine fut ciblée. Les facteurs excluant toute conversion étaient l'intervention chirurgicale ou une évidence de saignement. Pour tous les patients éligibles, le médecin traitant fut contacté à l'aide d'une recommandation écrite en vue de convertir le patient à un régime utilisant l'héparine non fractionnée.

RÉSULTATS: Durant une période de 10 mois, 463 patients furent identifiés recevant l'énoxaparine pour une thromboprophylaxie. De ceux-ci, 112 (24%) étaient des candidats à la conversion vers l'héparine non fractionnée. Un total de 88 recommandations de la pharmacie furent envoyées au médecin avec un taux d'acceptation de 67%. La conversion a permis d'éviter environ 250 jours de thérapie résultant en une épargne d'environ $8495 associés au coût du médicament.

CONCLUSIONS: Un programme visant la conversion d'une HBPM à une héparine non fractionnée pour une thromboprophylaxie peut résulter en une diminution significative des coûts de la médication tout en adhérant aux recommandations nationales.
