OBJECTIF: Réviser l'information actuellement disponible sur la combinaison atovaquone-proguanil dans la prophylaxie et le traitement de la malaria.

SOURCE DE DONNÉES: Un recherche sur MEDLINE de 1966 à février 2003 à l'aide des termes: malarone, atovaquone, proguanil, et malaria. D'autres articles sont identifiés au moyen des bibliographies primaires.

SÉLECTION DES ÉTUDES ET EXTRACTION DES DONNÉES: Les auteurs ont évalué les études animales et humaines de langue anglaise portant sur la pharmacologie, des études humaines de pharmacocinétique ainsi que des essais cliniques. Les données pertinentes en ont été extraites.

SYNTHÈSE DES DONNÉES: La combinaison atovaquone-proguanil a été étudiée à 8 reprises dans le traitement de malaria aiguë non-compliquée causée par le Plasmodium falciparum. Ces études ont démontré un taux de cure supérieur (87-100% vs. 72-88%) ou comparable (94-100% vs. 90.4-100%) aux anti-malariques classiques. De plus, l'atovaquone-proguanil a fait l'objet de 6 études en prophylaxie de la malaria. Quatre études contre placebo chez des populations semiimmunisées ou de voyageurs non-immuns ont démontré un taux d'efficacité variant entre 98 à 100% (48 à 82% pour le placebo). Deux études comparatives (versus chloroquine-proguanil ou mefloquine) ont démontré une efficacité prophylactique similaire pour les voyageurs sans immunité. Enfin, ces études cliniques ont démontré que la combinaison atovaquone-proguanil est généralement bien tolérée.

CONCLUSIONS: La combinaison atovaquone-proguanil est sécuritaire et est une alternative efficace aux prophylaxies et traitements actuels de la malaria.
