DRUG INFORMATION ROUNDS

OBJECTIF: Evaluer l'interaction pharmacocinétique entre le ténofovir et la didanosine employés en association antirétrovirale de haute efficacité.

REVUE DE LITTÉRATURE: Littérature repérée par une recherche sur MEDLINE (1966 à janvier 2003; mots-clés: ténofovir, didanosine). Les résumés de congrès récents, notamment ceux de la Société internationale du SIDA, de la Conférence internationale sur les agents antimicrobiens et la chimiothérapie (ICAAC), et de la Société américaine d'infectiologie, ont aussi été exploités.

RÉSUMÉ: Le ténofovir est autorisé dans le traitement de l'infection par le VIH en association avec d'autres agents antirétroviraux. L'interaction entre le ténofovir et la didanosine a été évaluée pour déterminer la sécurité de cette association.

CONCLUSIONS: Le ténofovir augmente significativement les concentrations plasmatiques et l'aire sous la courbe de la didanosine chez des volontaires sains. Des données supplémentaires sont nécessaires pour déterminer le mécanisme de cette interaction et la toxicité à long terme de cette association. Une réduction des doses de didanosine devrait être envisagée lorsque ces 2 médicaments sont employés concomitamment chez des patients infectés par le VIH.
