OBJECTIF: Décrire le cas d'une éruption cutanée de type psoriasique avec l'infliximab.

RÉSUMÉ DE CAS: Une femme âgée de 46 ans souffrant d'une fistule entérocutanée secondaire à la maladie de Crohn a développé des plaques érythémateuses, prurigineuses, et exfoliatives sur les coudes, les genoux, les mains, et les fesses après avoir reçu les deuxième et troisième doses d'infliximab intraveineux. L'examen histologique a démontré un tableau de dermatite lichénoïde. Il n'y a pas eu d'autres lésions cutanées après l'arrêt du traitement à l'infliximab.

DISCUSSION: De nombreuses réactions cutanées secondaires à l'infliximab, un anticorps monoclonal contre le TNF-alfa, ont été décrits dans la littérature. La dermatite de type psoriasique n'a pas été décrite comme réaction cutanée de l'infliximab, mais il existe plusieurs rapports de cas de dermatites de type psoriasique secondaires à d'autres médicaments. Selon un algorithme objectif de causalité, la réaction indésirable inpliquant l'infliximab est probable.

CONCLUSIONS: Nous rapportons un cas clinique s'étant manifesté par une éruption de type psoriasique et qui démontre un tableau histopathologique de dermatite lichénoïde. Ce modèle clinicopathologique dissocié de réaction cutanée n'a pas été attribué à un effet secondaire de l'infliximab.
