PEDIATRICS

OBJECTIF: Evaluer l'efficacité et l'innocuité de 3 inhibiteurs de la HMG-CoA réductase pour traiter la dyslipidémie associée au syndrome néphrotique de l'enfant.

REVUE DE LITTÉRATURE: Recherche informatisée sur les banques de données MEDLINE (entre 1966 et le mois d'avril 2004), Cochrane, et International Pharmaceutical Abstracts (entre 1977 et le mois d'avril 2004) d'articles pertinents en utilisant les mots-clés suivants: syndrome néphrotique, hypercholestérolémie familiale, dyslipidémie, et inhibiteurs de la HMG-CoA réductase.

SÉLECTION DES ÉTUDES: Deux essais prospectifs non contrôlés ont été évalués et inclus dans cette analyse de littérature scientifique.

RÉSUMÉ: Bien qu'une quantité importante de données soit disponible sur l'utilisation des inhibiteurs de la HMG-CoA réductase dans le traitement du syndrome néphrotique chez l'adulte, l'information est plutôt limitée quant á leur utilisation en pédiatrie. Selon les 2 essais pédiatriques disponibles, la réduction du cholestérol total, du cholestérol LDL, et des triglycérides suite á l'administration d'un inhibiteur de la HMG-CoA réductase peut atteindre jusqu'á 42%, 46%, et 44%, respectivement.

CONCLUSIONS: Les inhibiteurs de la HMG-CoA réductase ont démontré qu'ils constituent une option thérapeutique efficace et sécuritaire á court-terme pour contrôler la dyslipidémie associée au syndrome néphrotique. Des essais contrôlés de plus grande envergure sont toutefois requis pour valider ces résultats préliminaires.
