DRUG INFORMATION ROUNDS

OBJECTIF: Revoir la documentation scientifique portant sur l'utilisation de l'érythropoïétine dans le traitement de l'insuffisance cardiaque.

SOURCES D'INFORMATION: Les articles publiés dans des revues avec comités de pairs ont été identifiés par une recherche dans la banque de données MEDLINE. Les bibliographies de ces articles ont aussi été revues pour l'identification de références additionnelles.

RÉSUMÉ: La présence d'anémie assombrit le pronostic de l'insuffisance cardiaque congestive. Les études cliniques ont démontré que l'utilisation de l'érythropoïétine peut améliorer les symptômes de l'anémie chez les patients insuffisants cardiaques de classe fonctionnelle III ou IV dont le niveau d'hémoglobine est inférieur à 12 mg/dL. Des études de mortalité sont toutefois nécessaires pour confirmer à long terme ces bénéfices.

CONCLUSIONS: L'utilisation de l'érythropoïétine peut être utilisée de façon sélective chez certains patients souffrant d'insuffisance cardiaque sévère. Toutefois, des données additionnelles sont requises avant de pouvoir recommander d'emblée cette stratégie thérapeutique chez tous les patients anémiques souffrant d'insuffisance cardiaque.
