OBJECTIF: Rapporter un cas d'interaction entre le gingembre et le phenprocoumon ayant causé une augmentation du RNI et un épistaxis.

RÉSUMÉ DU CAS: Une Européenne de 76 ans a présenté un épistaxis associé à un RNI de 10 suite à une interaction entre le gingembre et le phenprocoumon.

DISCUSSION: Plusieurs études ont fourni des résultats contradictoires sur l'effet du gingembre sur l'hémostase et spécifiquement sur l'inhibition plaquettaire. Toutefois des recommandations ont été formulées à partir des données de ces études dans le but d'éviter l'utilisation du gingembre et d'autres produits naturels tels que l'ail et le ginkgo lors de situations où un saignement pourrait s'avérer critique. Une évaluation objective du lien de causalité a démontré que cette interaction était possible.

CONCLUSIONS: Ce rapport de cas est le premier à démontrer une interaction entre un anticoagulant oral et le gingembre. Une brève revue de littérature sur le gingembre et l'hémostase est également présentée. Comme l'interaction a été observée par hasard, ce cas fait aussi ressortir l'importance de l'auto-contrôle de l'anticoagulation avec les coumarines, particulièrement pour la détection de nouvelles interactions.
