OBJECTIF: Effectuer une revue de la littérature sur les déterminants, les corrélats, et les objectifs thérapeutiques d'une non adhésion au régime médicamenteux chez des personnes âgées demeurant à domicile.
SOURCES DE DONNÉES: Une recherche en langue anglaise dans les banques de données de MEDLINE, Pubmed et International Pharmaceutical Abstracts a été effectuée durant la période de Janvier 1966 à Décembre 2002. Les mots clés suivants ont été utilisés: personnes âgées, adhésion, non adhésion, compliance, non compliance, médicaments, méthodologie, déterminants, et hospitalisation.
SÉLECTION DES ÉTUDES ET EXTRACTION DES DONNÉES: Suite à la recherche informatique, les études concernant l'adhésion au régime médicamenteux ont été sélectionnées ainsi que les publications identifiées dans les listes de références des articles choisis.
ANALYSE DES DONNÉES: Même si plusieurs méthodes sont disponibles pour l'évaluation de l'adhésion au régime médicamenteux, des déterminants spécifiques sont difficiles à identifier. Les données actuelles suggèrent que la polymédication et la relation entre le patient et le soignant (incluant l'utilisation de plusieurs soignants) représentent des déterminants importants de la nonadhésion au régime médicamenteux chez les personnes âgées. Il n'existe pas de consensus quant aux autres déterminants de la non adhésion. Peu d'écrits ont examiné les associations entre la non adhésion et l'état de santé. Les données disponibles indiquent qu'une augmentation des problèmes de santé pourrait être notée suite à une non adhésion au régime médicamenteux. Cependant, des interventions spécifiques pour améliorer l'adhésion ont rarement démontré des résultats positifs sur l'état de santé.
CONCLUSIONS: Il existe peu de données scientifiques pour supporter une approche systématique chez les patients non observants. Des pistes de recherche seraient d'incorporer des évaluations des raisons spécifiques mentionnées par le patient suite à une non adhésion, le traitement approprié des médicaments et l'impact de la non adhésion sur l'état de santé.
