DRUG INFORMATION ROUNDS

OBJECTIF: Evaluer le rôle du cotrimoxazole comme alternative à la vancomycine pour le traitement des infections sévères à Staphylococcus aureus.

REVUE DE LITTÉRATURE: Littérature clinique repérée par MEDLINE et EMBASE. Mots-clés: association triméthoprime-sulfaméthoxazole, Staphylococcus aureus.

RÉSUMÉ: Il a été conduit une évaluation des rapports de cas, des séries, et des études cliniques relatives à l'emploi du cotrimoxazole dans le traitement des infections sévères à S. aureus. Le seul essai à répartition aléatoire a conclu à la supériorité de la vancomycine par rapport au cotrimoxazole.

CONCLUSIONS: Dans certaines infections à faible charge bactérienne, en particulier dans les endocardites droites ou les infections ostéoarticulaires, le cotrimoxazole est susceptible de représenter une alternative à la vancomycine pour le traitement des infections sévères à S. aureus. D'autres études à répartition aléatoire devraient être conduites pour comparer cet agent à la vancomycine et au linézolide.
