OBJECTIF: Décrire la présence d'un infarctus aigu du myocarde suite à la prise d'un supplément diététique contenant de Citrus aurantium L. (Petit grain bigarade ou Bitter Orange) chez une patiente sans antécédent de problèmes cardiovasculaires.
PRÉSENTATION DU CAS: Il s'agit d'une femme de 55 ans qui se présente à l'urgence avec des symptômes de douleurs rétro sternales et une douleur à l'épaule droite. Une revue de ses médicaments a révélé que la patiente avait ingéré durant la dernière année un supplément diététique pourperte de poids qui contenait 300 mg de Bitter Orange (Edita's Skinny Pill). Même si ses antécédents médicaux ne révélaient pas d'hypertension, de maladie coronarienne, ou d'hyperlipidémie, son artériographie a révélé une lésion dans l'artère coronaire gauche. Elle avait une histoire de tabagisme. Un diagnostic d'infarctus du myocarde a été posé et la patiente hospitalisée pendant 4 jours.
DISCUSSION: Les consommateurs considèrent les suppléments diététiques sécuritaires. Cependant, certains suppléments pour perte de poids contiennent des ingrédients qui ont été associés à des problèmes cardiovasculaires. Selon l'échelle de Naranjo, le C. aurantium est possiblement associé à l'infarctus du myocarde présenté par la patiente.
CONCLUSIONS: La prise de produits diététiques contenant du C. aurantium peut représenter un facteur de risque pour développer une toxicité cardiovasculaire. Cependant, d'autres études devront être effectuées pour confirmer cette observation.
