OBJECTIF: Rapporter un cas d'intoxication mixte à la venlafaxine et à la quétiapine causant de multiples complications dont des convulsions, un coma prolongé, une dépression respiratoire, un syndrome neuroleptique malin, des intervalles QRS et QT prolongés, et un possible syndrome de retrait à la venlafaxine.

SOMMAIRE DU CAS: Un homme de 53 ans a été admis à l'hôpital pour une perte de conscience secondaire à une intoxication volontaire à la venlafaxine et à la quétiapine.

DISCUSSION: La quétiapine pourrait être responsable pour le syndrome neuroleptique malin. De plus, étant donné la présence d'une intoxication à la venlafaxine, de fièvre, d'instabilité autonomique, et de myoclonies, un diagnostic différentiel de syndrome sérotoninergique a également été proposé. Les causes potentielles de convulsions et d'allongement des intervalles QRS et QT sont révisées. Finalement, le coma prolongé ainsi que le syndrome de retrait à la venlafaxine sont discutés en insistant sur les paramètres pharmacodynamiques et pharmacocinétiques des 2 médicaments dans le contexte d'une intoxication.

CONCLUSIONS: Ce rapport décrit les défis cliniques d'une intoxication mixte à la venlafaxine et à la quétiapine. Les cliniciens devraient être vigilants quant aux possibles complications survenant à la suite d'une intoxication mixte aux antidepresseurs et aux antipsychotiques atypiques.
