AMBULATORY CARE

OBJECTIF: Faire le point sur l'efficacité des macrolides et des tétracyclines dans plusieurs affections inflammatoires chroniques.

SOURCES DE DONNÉES: Des recherches dans MEDLINE (1966– mars 2004) ainsi qu'une recherche bibliographique étendue ont été entreprises. Les mots-clés comprenaient acné, blépharite, maladie cardiovasculaire, mucoviscidose, périodontite, couperose, et polyarthrite rhumatoïde.

SYNTHÈSE DES DONNÉES: Les antibiotiques sont souvent prescrits pendant des mois voire des années pour des affections inflammatoires chroniques telles que acné, blépharite, maladie cardiovasculaire, mucoviscidose, périodontite, couperose, et polyarthrite rhumatoïde. Des essais croisés randomisés ont montré que l'azithromycine est utile dans la prise en charge de la mucoviscidose, et que les tétracyclines sont bénéfiques dans celle de la polyarthrite rhumatoïde, l'acné, la blépharite, et la périodontite. Plusieurs essais contrôlés randomisés importants n'ont pas réussi à montrer de bénéfices des macrolides dans la prévention secondaire des maladies cardiovasculaires. Aucun essai randomisé contrôlé contre placebo n'a été réalisé pour établir l'efficacité des macrolides ou des tétracyclines chez les patients atteints de couperose.

CONCLUSIONS: L'utilisation des tétracyclines et des macrolides dans la couperose repose surtout sur des rapports de cas anecdotiques ou des essais en ouvert. Des essais cliniques limités soutiennent l'utilisation des tétracyclines ou des macrolides dans l'acné, la blépharite, la polyarthrite rhumatoïde, et la mucoviscidose. Pour l'instant, les essais ne soutiennent pas l'usage des antibiotiques dans la prévention secondaire des maladies cardiovasculaires.
