FORMULARY FORUM

OBJECTIF: Cet article revoit l'épidémiologie, la pathogénèse, et la prise en charge des patients atteints de déficience en alpha-antitrypsine (AAT). Une demande d'addition au formulaire minutieuse de l'Aralast a été effectuée en comparant les données publiées à celles disponibles sur le traitement de premier choix - Prolastin - 1 des 2 AAT obtenues à partir du plasma humain.

SOURCES DES DONNÉES: Les articles ont été identifiés par une recherche MEDLINE en utilisant les mots-clés {alpha}-antitrypsin et emphysema.

SÉLECTION DES ÉTUDES ET DONNÉES: Tous les articles publiés dans des journaux révisés par les pairs portant sur l'efficacité in vitro ou clinique de l'AAT obtenue du plasma (eg, Prolastin, Aralast) chez les patients atteints de ce désordre autosomal récessif.

EXTRACTION ET SYNTHÈSE DES DONNÉES: Les données révisées par les pairs sur toutes les AAT ont été évaluées, particulièrement les essais cliniques.

CONCLUSIONS: Les essais cliniques visant à augmenter l'AAT préviennent la progression de l'emphysème relié à la déficience de l'AAT et la morbidité et mortalité qui y sont associées. Le traitement avec l'Aralast a été démontré sûr et bien toléré avec une faible incidence d'effets indésirables d'intensité faible à modérée. Les études pharmacoéconomiques sur l'augmentation de l'AAT démontrent que l'Aralast est coût-efficace comme thérapie visant à augmenter la survie chez les patients atteints d'emphysème relié à la déficience de l'AAT. Sur la base de ces données, l'Aralast devrait être évaluée pour addition au formulaire.
