NEW DRUG APPROVALS

OBJECTIF: Faire le point sur le pémétrexed, un nouvel agent multicible anti-folates.

REVUE DE LITTÉRATURE: Littérature repérée par des recherches sur MEDLINE et CANCERLIT (1985 à septembre 2004). Des résumés récents de la Société américaine d'oncologie clinique (ASCO) ont aussi été inclus, ainsi que de l'information fournie par l'industriel. Les mots-clés étaient pémétrexed, LY-231514, Alimta, anti-folate multicible, et mésothéliome pleural malin.

SÉLECTION DES ÉTUDES ET DE L'INFORMATION: Il a été sélectionné l'information pertinente sur la pharmacologie et la pharmacocinétique du pémétrexed ainsi que sur sa sécurité et son efficacité à partir des essais cliniques.

RÉSUMÉ: Le pémétrexed inhibe le métabolisme des folates et la synthèse des bases puriques et pyrimidiniques. Selon les essais de phases 1 et 2, le pémétrexed présente une activité antitumorale dans les tumeurs solides, notamment pulmonaires, colorectales, et cervicales. Une étude pivot de phase 3, réalisée chez des patients atteints de mésothéliome pleural malin, a montré une amélioration de la survie avec des schémas pémétrexed + cisplatine versus cisplatine seul.

CONCLUSIONS: Les auteurs estiment que le pémétrexed est un nouveau médicament prometteur pour le traitement des cancers solides, notamment le mésothéliome pleural.
