ADHERENCE

OBJECTIF: Documenter le gaspillage de médicaments en utilisant un système réutilisable d'aide à la prise de médicaments remplis par la pharmacie et explorer en utilisant une approche qualitative la communication entre les médecins et pharmaciens concertant le gaspillage de médicaments.

MÉTHODES: Un nombre de 62 patients âgés de plus de 60 ans et demeurant dans foyers-logements ont participé à cette étude. Les patients dans le groupe intervention ont reçu leurs médicaments dans un système d'aide à la prise de médicaments tandis que les participants du groupe contrôle ont reçu leurs médicaments dans un emballage traditionnel. Les pharmaciens communautaires ont documenté le gaspillage des médicaments durant la période de l'étude (3 mois) et pour le groupe intervention à 6, 9, et 12 mois, respectivement, post-étude. Les entrevues qualitatives ont été menées auprès des médecins de famille et pharmaciens communautaires pour évaluer leurs opinions quant à l'utilisation de système d'aide à la prise aux médicaments.

RÉSULTATS: Dans le groupe intervention, le gaspillage des médicaments a diminué de 18% à 1%. Aucune donnée n'a été documentée avec le groupe contrôle après la phase d'évaluation sur le gaspillage. Les auteurs rapportent que dans le groupe intervention, il y avait 61% plus de changements dans les prescriptions incluant une diminution dans le nombre de médicaments prescrits et des modifications au niveau de la posologie et de la dose. Les médecins de famille et les pharmaciens ont rapporté une meilleure communication concernant les modifications apportées au profil médicamenteux des patients.

CONCLUSIONS: Cette étude exploratoire a démontré que l'organisation d'un système à la prise des médicaments a permis de comprendre les éléments qui pouvaient favoriser le gaspillage des médicaments.
