DRUG INFORMATION ROUNDS

OBJECTIF: Evaluer le rôle du monoxyde d'azote inhalé chez des greffés cardiaques ou pulmonaires.

REVUE DE LITTÉRATURE: Littérature repérée par une recherche sur MEDLINE (1966 à juillet 2004).

RÉSUMÉ: L'hypertension pulmonaire conduisant à l'insuffisance cardiaque droite et les défauts de reperfusion après ischémie sont des complications respectives des transplantations cardiaque et pulmonaire. Cette revue de 7 essais publiés analyse le rôle du monoxyde d'azote chez ces populations et donne des recommandations basées sur la littérature sur le traitement, la posologie, et la surveillance.

CONCLUSIONS: Le monoxyde d'azote présente des effets hémodynamiques bénéfiques dans les suites post-opératoires immédiates chez des greffés cardiaques et des effets hémodynamiques variables chez des greffés pulmonaires. Les données de morbi-mortalité disponibles ne sont pas en faveur de ces indications: il s'agit d'un traitement symptomatique qui nécessite d'autres études.
