ADVERSE DRUG REACTIONS

CONTEXTE: Plusieurs effets indésirables sont le résultat d'effets pharmacologiques connus et d'autres, d'erreurs médicamenteuses. Cependant, certains sont reliés à des risques spécifiques aux patients.

OBJECTIF: Identifier les facteurs de risques des effets indésirables aux médicaments (EIM) chez une clientèle hospitalisée.

MÉTHODES: Une régression logistique conditionnelle a servi à analyser toutes les EIM vérifiées par un pharmacien, les EIM par classe thérapeutique et leur sévérité sur une période de 10 ans. Tous les patients de plus de 18 ans hospitalisés dans un centre universitaire de soins tertiaires ont été inclus. Chaque cas-patient a été jumelé jusqu'à 16 castémoins. Les rapports de cotes (RC) pour les facteurs de risques associés aux EIM ont été calculés pour chacune des classes thérapeutiques.

RÉSULTATS: Les RC de plusieurs facteurs de risques ont été identifiés pour 4376 EIM et varient en fonction de la classe thérapeutique. Les facteurs de risques pour différents médicaments ont été groupés selon: caractéristiques du patient—sexe féminin (RC 1.5–1.7), âge (0.7–0.9), poids (1.2–1.4), clairance à la créatinine (0.8–4.7), nombre de comorbidités (1.1–12.6); administration du médicament—dose (1.2–3.7), voie d'administration (1.4–149.9), nombre de médicaments concomitant (1.2–2.4); patient—service (1.2–4.9), division des soins infirmiers (1.5–3.8) et les diagnostics regroupés pour la gestion (DRG) (1.5–5.7).

CONCLUSIONS: Certains facteurs de risques sont présents pour tous les EIM et à travers plusieurs classes thérapeutiques alors que certains sont spécifiques à certaines classes. Les médicaments à risque élevé devraient faire l'objet d'un suivi plus étroit en fonction des caractéristiques du patient (genre, âge, poids, clairance à la créatinine et comorbidités) et de l'administration du médicament (dose, voie et nombre de médicaments concomitants).
