OBJECTIF: Décrire l'état de santé des nouveau-nés dont les mères prenaient de l'azathioprine durant l'allaitement et déterminer la quantité de 6-mercaptopurine (6-MP), le métabolite actif de l'azathioprine, dans le lait maternel de ces mères.

RÉSUMÉ DU CAS: Cet article résume le cas de 4 patientes traitées avec de l'azathioprine durant l'allaitement de leur nouveau-né. Les échantillons de lait maternel ont été analysés pour le 6-MP chez 2 de ces 4 patientes. L'analyse des concentrations de 6-MP par une technique de chromatographie à haute pression en phase liquide a démontré des niveaux indétectables de 6-MP. Considérant le niveau de détection du 6-MP établi à 5 ng/mL, la dose relative absolue chez l'enfant aurait été moins de 0.09% de la dose maternelle ajustée pour le poids. Aucun effet indésirable n'a été noté chez les 4 nouveau-nés.

DISCUSSION: Une proportion importante de femmes en âge de procréer souffrent de conditions médicales chroniques telles le lupus érythémateux ou la transplantation d'organes solides et requièrent une immunosuppression qui peut inclure l'azathioprine. Tout comme les autres médicaments antimétabolites, l'azathioprine est contre-indiquée durant l'allaitement pour des considérations théoriques de toxicité chez le nouveau-né. La littérature à cet effet est limitée et date d'un certain nombre d'années. Les données des 4 cas présentées dans cet article révèlent que le 6-MP est excrété de façon minimale dans le lait maternel. Les répercussions cliniques sur le nouveau-né semblent donc minimales.

CONCLUSIONS: L'utilisation de l'azathioprine par une mère allaitante ne semble pas poser de risques cliniques immédiats au nouveau-né. Le suivi à long-terme de ces nouveau-nés est toutefois recommandé.
