PSYCHIATRY

OBJECTIF: Procéder à une révision de la sécurité et de la tolérabilité des nouveaux agents utilisés pour traiter la maladie bipolaire et fournir aux cliniciens des stratégies de gestion des effets indésirables de ces agents.

REVUE DE LITTÉRATURE: Les mots clés suivants ont été utilisés pour questionner la banque de données Medline des premières entrées jusqu'au mois de juillet 2005: bipolar disorder treatment, adverse effects, tolerability, safety, emerging agents, atypical antipsychotics, new antiepileptic drugs, risperidone, quetiapine, clozapine, ziprasidone, aripiprazole, lamotrigine, topiramate, gabapentin, oxcarbazepine, and olanzapine.

SÉLECTION DES ÉTUDES: Les données rapportées proviennent d'études randomisées, d'études contrôlées, d'études ouvertes ainsi que de revues de traitement.

RÉSUMÉ: Les agents émergeants récemment approuvés pour le traitement de la maladie bipolaire incluent des antipsychotiques atypiques, et de nouveaux anticonvulsivants. La sécurité et la tolérabilité de ces agents sont aussi importantes que leur efficacité car la faible adhésion des patients bipolaires à leur traitement limite le succès de la thérapie. De plus, les comorbidités métaboliques et autres compliquent le traitement et les patients maniaques requièrent souvent des combinaisons d'agents ayant pour effet d'augmenter les effets indésirables.

CONCLUSIONS: La plupart des antipsychotiques atypiques causent moins de réactions extrapyramidales que les antipsychotiques conventionnels mais entraînent plus de prise de poids et de complications métaboliques. Les nouveaux anticonvulsivants causent généralement moins de prise de poids que les anciens et certains peuvent même occasionner une perte de poids. Certains anticonvulsivants plus récents utilisés dans la maladie bipolaire sont avantageux car ils présentent moins d'interactions et ne nécessitent pas de suivis des concentrations sériques. Dépendamment des résultats attendus d'études contrôlées en cours, certains agents émergeants pourront se révéler des ajouts intéressants à l'arsenal thérapeutique de la maladie bipolaire.
