DRUG INFORMATION ROUNDS

OBJECTIF: Evaluer l'emploi du bleu de méthylène dans le traitement de l'encéphalopathie induite par l'ifosfamide.

SOURCES DES DONNÉES: Recherches sur MEDLINE (1966 à août 2005) et International Pharmaceutical Abstracts (1971 à août 2005), avec les mots-clés methylene blue, ifosfamide, encephalopathy, et neurotoxicity.

SYNTHÈSE DES DONNÉES: L'utilisation du bleu de méthylène dans le traitement de l'encéphalopathie induite par l'ifosfamide a été décrite dans plusieurs rapports de cas anecdotiques et dans une analyse rétrospective de dossiers mais aucun essai clinique contrôlé n'a été retrouvé. Chez quelques patients, le bleu de méthylène s'est révélé contribuer à la disparition des symptômes d'encéphalopathie dans un délai de 10 minutes après administration, mais il a une efficacité modeste dans la plupart des cas. Chez les autres patients, les symptômes ont disparu dans un délai compatible avec une disparition de l'encéphalopathie induite par l'ifosfamide sans traitement.

CONCLUSIONS: Les données disponibles provenant de rapports de cas anecdotiques indiquent que le bleu de méthylène est une option dans le traitement de l'encéphalopathie induite par l'ifosfamide, en particulier chez les patients présentant des symptômes sévères de toxicité. Cependant, l'absence d'essais cliniques contrôlés et la possibilité d'une disparition spontanée de l'encéphalopathie ne permettent pas d'apprécier l'utilité du bleu de méthylène.
