DRUG, ALCOHOL, AND SUBSTANCE ABUSE

OBJECTIF: Revoir l'évidence clinique déterminant l'efficacité et l'innocuité de l'acide valproïque dans le traitement du syndrome de retrait alcoolique (SRA).

REVUE DE LITTÉRATURE: Une recherche informatisée effectuée sur MEDLINE (1966 à février 2006), EMBASE (1980 à février 2006), et PubMed (1966 à février 2006) a permis l'identification d'articles de recherche clinique de langue anglaise. Les mots clés utilisés sont valproic acid, ethanol, alcohol, alcoholism, alcohol withdrawal delirium, alcohol withdrawal seizures, et substance withdrawal syndrome. Les références d'articles identifiés ont été évaluées manuellement.

SÉLECTION DES ÉTUDES ET DE L'INFORMATION: Toutes les études cliniques contrôlées chez les humains ayant évalué l'utilisation de l'acide valproïque dans le traitement du SRA ont été inclues.

RÉSUMÉ: Les études ont comparé divers régimes d'acide valproïque au traitement traditionnel avec benzodiazépines ou des agents autres que les benzodiazépines. Seules, 2 des 6 études ont rapporté une différence significative en faveur de l'acide valproïque par rapport à certains paramètres du SRA. Cependant, ces différences étaient d'une conséquence clinique peu importante. Le nombre de patients inclus dans ces études n'a pas permis une évaluation adéquate de son innocuité.

CONCLUSIONS: Les données existantes sur l'efficacité et l'innocuité de l'acide valproïque suggèrent que ce médicament ne devrait pas remplacer la thérapie conventionnelle ou ne devrait pas être utilisé à titre de thérapie adjuvante dans le traitement de SRA léger à modéré.
