SURGERY

OBJECTIF: Évaluer l'innocuité des antagonistes à 5-HT3 en monothérapie ou en combinaison avec un stéroïde ou dropéridol pour la prophylaxie des nausées et vomissements postopératoires (NVPO).

REVUE DE LITTÉRATURE: Les études contrôlées et randomisées (ECR) furent identifiées via une recherche MEDLINE couvrant la période de 1966 à septembre 2005 et utilisant les mots clés anglais suivants: 5-HT3, granisetron, ondansetron, dolasetron, tropisetron, PONV, postoperative, vomiting, emesis, et nausea.

SÉLECTION DES ÉTUDES ET DE L'INFORMATION: Les ECR dont l'un des bras comparait un antagoniste à 5-HT3 (granisetron, ondansetron, dolasetron, ou tropisetron) à la dexaméthasone ou au dropéridol ou encore à une combinaison ont été sélectionnées. L'innocuité fut également évaluée.

MÉTHODOLOGIE: Des rapports de cotes (RC) avec intervalle de confiance à 95% (IC) furent calculés pour les incidences d'effets secondaires d'un antagoniste à 5-HT3 en monothérapie ou des thérapies en combinaison. L'ampleur de l'effet pour les effets secondaires fréquemment rapportés fut estimée en additionnant les RC selon un modèle fixe ou aléatoire.

RÉSULTATS: Les RC pour les effets secondaires de la combinaison 5-HT3/dexaméthasone versus 5-HT3 seul pour la prophylaxie de NVPO ne furent significatifs pour aucun effet secondaire ni pour l'incidence totale d'effets secondaires; la comparaison de la combinaison 5-HT3/dropéridol versus le 5-HT3 en monothérapie fut significative seulement pour l'incidence réduite de céphalées (RC 0.35; IC0.18 à 0.69). Le RC pour la combinaison 5-HT3/dexaméthasone versus dexaméthasone seule ne fut significative que pour les céphalées (RC 1.75; IC1.01 à 3.03). Le RC pour la combinaison 5-HT3/dropéridol versus le dropéridol en monothérapie ne fut significatif pour aucun effet secondaire. Aucun rapport de nécrose avasculaire, d'infection occulte ou de retard de cicatrisation ne fut observé. Des anomalies cardiaques furent observées avec la combinaison 5-HT3/dropéridol.

CONCLUSIONS: Cette méta-analyse démontre que chaque thérapie possède une innocuité similaire à la dexaméthasone, au dropéridol ou à une thérapie à un antagoniste à 5-HT3.
