OBJECTIF:Rapporter un cas d'empyème à Streptococcus intermedius secondaire à une pneumonie acquise en communauté.

RÉSUMÉ DU CAS:Un empyème s'est développé chez une patiente de 85 ans, connue pour des antécédents de maladie pulmonaire obstructive chronique, d'asthme, d'insuffisance cardiaque et d'hypothyroïdie, à la suite de 2 pneumonies acquises en communauté (PAC) et d'une exacerbation aigue d'une bronchite chronique en l'espace de 2 mois. Les cultures bactériennes de l'empyème révèlent une culture pure de S. intermedius. Malgré un traitement agressif incluant une décortication chirurgicale, la patiente est décédée après 30 jours d'hospitalisation.

DISCUSSION:Un traitement antimicrobien rapide et approprié est à la base de la prise en charge de la PAC. Toutefois rares, l'empyème et l'abcès thoracique sont des complications possibles de l'infection. Historiquement, les antibiotiques utilisé pour les PAC ont une activité contre le S. intermedius, mais la croissance de cet organisme dans l'espace pleural de cette patiente doit alerter le clinicien des échecs possibles.

CONCLUSIONS:Les auteurs rapportent un cas d'empyème à S. intermedius secondaire à une pneumonie acquise en communauté qui, malgré des données de sensibilités historiques, ont nécessité des mesures chirurgicales. Une antibiothérapie rapide demeure la base du traitement de la PAC et aide à prévenir les complications fatales.
