Efficacité d'un Système Électronique pour Déclarer les Effets Indésirables des Médicaments Intégré au Système Informatique de l'Hôpital

OBJECTIF: Évaluer l'efficacité d'un système électronique pour déclarer les effets indésirables des médicaments intégré au système informatique de l'hôpital (ADR-RS-IHIS). Proposer et mettre en place des améliorations à la déclaration des effets indésirables aux médicaments et évaluer l'impact ce programme.

MÉTHODES: Chacun des effets indésirables des médicaments documentés via le ADR-RS-IHIS a été évalué rétrospectivement. Deux phases ont été identifiées: la phase I s'est déroulée du mois d'avril 2004–août 2006 et a évalué l'efficacité du nouveau programme. En avril 2006, une analyse intérimaire a été effectuée pour proposer des améliorations au programme. La phase II s'est déroulée de septembre 2006–avril 2007 et à évaluer l'impact des améliorations proposées. L'efficacité de la phase I et l'impact des améliorations ont été mesurés comme étant le nombre d'effets indésirables des médicaments rapporté au programme national de pharmacovigilance.

RÉSULTATS: Le taux de documentation des effets indésirables aux médicaments rapporté par mois au programme national de pharmacovigilance a augmenté de zéro, avant 2004, à 0.91 dans la phase I et de 1.62 dans la phase II. Les mesures d'amélioration étaient les suivantes: déclaration par les infirmières au même titre que les pharmaciens et les médecins, documentation automatique de certaines informations dans le formulaire provenant du système électronique de l'hôpital, rapidité de l'analyse des déclarations, et notification automatique des allergies potentielles.

CONCLUSIONS: Un système électronique pour déclarer les effets indésirables des médicaments intégré au système informatique de l'hôpital est efficace pour augmenter le nombre de déclarations d'effets indésirables aux médicaments au programme national de pharmacovigilance. Les déclarations effectuées par les infirmières sont comparables à celles effectuées par les médecins et le formulaire électronique contribue à l'amélioration du système.
