Les Options Thérapeutiques dans la Leucémie Myéloïde Chronique Résistante à l'Imatinib.

OBJECTIF: Passer en revue les nouvelles options thérapeutiques disponibles pour le traitement de la leucémie myéloïde chronique (LMC) chez les patients intolérants ou ayant subi un échec à l'imatinib.

REVUE DE LITTÉRATURE: Une revue de la documentation scientifique a été effectuée à partir des ressources suivantes: MEDLINE (1966-mai 2007), EMBASE (1991-2007), Proceedings de la Société américaine d'hématologie (2000-2006), et Proceedings de la Société américaine d'oncologie clinique (2000-2007). Les mots-clés incluaient: imatinib, desatinib, nilotinib, et leucémie myéloïde chronique. Les résumés de congrès et les études de langue anglaise rapportant les résultats d'essais précliniques et cliniques de phases 1, 2, et 3 ont été retenus pour les fins de cet article.

RÉSUMÉ: L'imatinib constitue le traitement de choix de la LMC; toutefois, certains patients peuvent présenter une résistance ou une intolérance au médicament. Des données d'études cliniques de phases I et II portant sur des inhibiteurs plus puissants de la tyrosine kinase (desatinib et nilotinib) sont prometteuses. Des réponses hématologiques et cytogénétiques sont rapportées pour ces 2 médicaments. Il ne semble pas exister de résistance croisée entre ces médicaments bien qu'aucun ne soit efficace contre toutes les mutations du marqueur moléculaire de la LMC, le chromosome de Philadelphie. De nouveaux médicaments sont aussi en investigation pour le traitement de ces patients, incluant notamment les inhibiteurs de l'aurora kinase et de la farnésyl transférase ainsi que des associations médicamenteuses.

CONCLUSIONS: Le desatinib et le nilotinib sont des options de second recours pour les patients avec LMC et qui sont résistants ou intolérants à l'imatinib. Les effets indésirables de ces différents traitements peuvent différer. Des études cliniques portant sur ces médicaments ainsi que sur d'autres traitements sont en cours.
