Les Nomogrammes Posologiques: Silos sur un Pente

Le développement de nomogrammes posologiques a plusieurs avantages, en particulier pour standardiser les soins. Il est fréquent pour un clinicien de devoir prendre en compte de multiples facteurs dans le cadre d'une gestion individualisée du patient. Dans certains cas, une approche préétablie et standardisée peut créer des barrières à l'individualisation de la thérapie médicamenteuse, coinçant potentiellement les patients à l'intérieur de catégories posologiques (silos) et minimisant ainsi les besoins individuels. Les différentes approches utilisées pour les inhibiteurs directs de la thrombine ont été revues récemment et des posologies différentes de celles recommandées par le fabricant ont ainsi été proposées. Ce commentaire explore les considérations à prendre en compte dans l'anticoagulothérapie et discute des guides de pharmacothérapie et de monitorage.
