Critères STOPP et START: Fiabilité Interjuge Entre Pharmaciens

OBJECTIF: Déterminer la fiabilité interjuges parmi un groupe de pharmaciens cliniciens dans l'application des critères STOPP (outil pour évaluer les médicaments chez les personnes âgées) et START (outil pour informer les médicaments d'un traitement approprié) en utilisant les profils médicamenteux de personnes âgées.

MÉTHODES: Dix pharmaciens (5 pharmaciens oeuvrant en milieu hospitalier et 5 pharmaciens oeuvrant en milieu communautaire) ont reçu chacun 20 profils de patients avec les informations suivantes: l'âge et le sexe du patient, les médicaments et les listes de problèmes actifs, les informations pertinentes du dossier antérieur, les résultats des valeurs de laboratoire, et la valeur estimée de la filtration glomérulaire. Chaque pharmacien a appliqué les critères STOPP et START à chacun des profils médicamenteux qu'il devait évaluer. Les médicaments potentiellement inappropriés et les erreurs potentiels d'omission identifiés par chacun des pharmaciens ont été comparés à ceux de 2 pharmaciens oeuvrant dans un milieu universitaire qui connaissaient très bien les critères STOPP et START. Une analyse de la fiabilité interjuge en utilisant les coefficients kappa a été effectuée afin de déterminer la cohérence des données entre les pharmaciens.

RÉSULTATS: Les coefficients {kappa} médian pour les pharmaciens hospitaliers et les pharmaciens communautaires lorsque comparé aux pharmaciens universitaires pour les critères STOPP étaient de 0.89 et 0.88 respectivement, tandis que ceux de START étaient de 0.91 et 0.90 respectivement.

CONCLUSIONS: La fiabilité interjuge des critères STOP et START entre les pharmaciens oeuvrant dans différents secteurs est bonne. Les pharmaciens travaillant soit en milieu hospitalier ou communautaire peuvent utiliser les critères STOPP et START durant leur pratique quotidienne afin d'identifier les médicaments potentiellement inappropriés ou les erreurs potentiels d'omission.
