TRASFONDO: Flavopiridol, un derivado nuevo de flavona, inhibe la quinina-I dependiente de ciclina. Iniciamos un estudio de Fase I en pacientes con tumores sólidos refractarios para determinar la dosis máxima tolerable y caracterizar el perfil de efectos secundarios.

OBJETIVO: Caracterizar la farmacología clínica de flavopiridol.

MÉTODOS: Se recolectaron muestras plasmáticas en serie y se analizaron por HPLC usando detección electroquímica. Para la farmacocinética, se utilizó un análisis no-compartimentar. La recirculación enterohepática fue estudiada analizando muestras fecales en un intento de correlacionar la colescistoquinina y los picos post-infusión. El enlace a las proteínas del plasma, se estudió usando diálisis de equilibrio.

RESULTADOS: Setenta y seis pacientes fueron tratados con flavopiridol a 13 niveles de dosis para un total de 504 ciclos de tratamiento. La concentración promedio de estado estacionario fue de 26.5 y 253nM a 4 y 122.5 mg/m2, respectivamente. La depuración varió de 49.9 a 2943 mL/min, demostrando no-linealidad a dosis mayores de 50 mg/m2/día. Un aumento en concentración plasmática de flavopiridol después de la infusión se encontró en un grupo de pacientes, generalmente entre las 3 y 24 horas después de terminada la infusión. Flavopiridol fue encontrado en la materia fecal lo que sugiere recirculación enterohepática. Hubo enlace no saturable de flavopiridol a proteínas del plasma (fu = 6%).

CONCLUSIONES: La toxicidad limitante de dosis para el estudio de Fase I de flavopiridol fue diarrea secretoria. No pudimos identificar una relación clara entre la dosis o la concentración y la diarrea. A dosis de 50 y 78 mg/m2/d, la concentración promedio de estado estacionario fue de 278 y 390 nM. Estas concentraciones estaban bastante por encima de lo que se utiliza en estudios in vitro para actividad antiproliferativa.
