OBJETIVO: Revisar la información disponible sobre la combinación aspirina/warfarina versus aspirina sola para la prevención secundaria luego de un infarto al miocardio.

FUENTES DE INFORMACION: Se identificó la literatura utilizando MEDLINE (1966–septiembre 2002). Los términos de la búsqueda incluyeron aspirina, warfarina, prevención secundaria, e infarto al miocardio.

SINTESIS: A pesar del uso de dosis bajas de aspirina post-infarto al miocardio, el riesgo de muerte subsiguiente y de eventos isquémicos continúa alto lo que hace imperativo que se desarrollen estrategias para prevención secundaria. Se evaluaron estudios relevantes, grandes y a largo plazo enfocando en terapia dual de aspirina/warfarina versus aspirina solamente en pacientes post-infarto al miocardio.

CONCLUSIONES: Aspirina 75–325 mg debe continuar como la terapia de primera línea en prevención post-infarto al miocardio. La combinación de aspirina 75–81 mg con warfarina (INR 2–2.5) podría proveer beneficio adicional pero sólo debe ser considerada en pacientes con un alto riesgo de eventos tromboembólicos.
