ADVERSE DRUG REACTIONS

OBJETIVO: Revisar y analizar la literatura médica documentando daño esofágico y la disfagia inducidos por drogas. Formular estrategias para fomentar el papel del farmaceútico en la prevención, detección, y tratamiento de estas complicaciones iatrogénicas.

FUENTES DE INFORMACIÓN: Se realizó una búsqueda MEDLINE (1966–abril 2002) para identificar literatura primaria y secundaria pertinente. Se utilizaron varias combinaciones de las siguientes palabras claves: pill-induced, (inducido por píldoras) drug-induced (inducido por drogas), o iatrogenic con esophageal injury, esophageal damage, o dysphagia (iatrogénico con daño esofágico o disfagia). Se evaluaron las bibliografías de estos artículos para identificar artículos adicionales.

SELECCIÓN DE ESTUDIOS Y EXTRACCIÓN DE DATOS: Se evaluaron todos los informes, artículos de repaso, y estudios clínicos relacionados con daño esofágico inducido por drogas y dificultad al tragar.

SÍNTESIS: Daño esofágico inducido por drogas no es debidamente reconocido. Se ha asociado el uso de varias drogas con las heridas causadas por efectos físicos o químicos y se han identificado factores de riesgo. Ciertas estrategias preventativas y ciertos tratamientos han mostrado éxito en limitar el daño esofágico. La disfagia inducida por drogas puede tener complicaciones serias y es comúnmente asociada con el uso de neurolépticos típicos como el haloperidol.

CONCLUSIONES: Los farmaceúticos pueden jugar un papel importante en la identificación de antemano de situaciones en las cuales hay una alta posibilidad de daño esofágico o disfagia inducidos por drogas. Los farmaceúticos pueden recomendar estrategias preventativas para promover el uso apropiado de medicaciones, ayudar a identificar estas complicaciones iatrogénicas cuando ocurren, y asisitir en la formulación de estrategias de tratamiento apropiadas.
