DRUG INFORMATION ROUNDS

OBJETIVO: Revisar el rol de alopurinol como cardioprotector durante una cirugía de puente aorto coronario (CABG por sus siglas en inglés).

EXTRACCIÓN DE DATOS: Una búsqueda en MEDLINE (1966–octubre 2002) fue realizada usando los siguientes términos: alopurinol, oxidasa de xantina, radical de oxígeno libre, y puente aorto coronario.

SELECCIÓN DE ESTUDIOS: La cantidad de citas bibliográficas recuperadas en la búsqueda con los términos: alopurinol, xantina oxidasa, y radical de oxígeno libre fueron independientemente combinadas con la cantidad de citas bibliográficas del término puente aorto coronario limitadas al lenguaje inglés y estudios en humanos. Se obtuvieron 41 citaciones, y éstas fueron evaluadas por su adaptabilidad al tópico. Trece estudios fueron encontrados adecuado con todos los resultados positivos. Cinco estudios de gran escala son discutidos.

CONCLUSIONES: Alopurinol podría reducir la incidencia de complicaciones en la cirugía de puente aorto coronario. A pesar de que la dosis óptima no ha sido determinada, la literatura revisada sugiere, al menos, que los pacientes deberían recibir alopurinol 600 mg el día antes y el día de la cirugía.
