OBJETIVO: Evaluar la relación entre la frecuencia de base de convulsiones con una dosis estable de topiramate y el efecto de tratamiento individualizado según la tolerabilidad en adultos con epilpepsia de tipo parcial recibiendo otros agents anti-epilépticos.

MÉTODOS: En este estudio de etiqueta abierta de 20 semanas de duración, las dosis de los medicamentos fueron ajustadas según la respuesta clínica. Se analizaron las dosis y la información en cuanto a respuesta clínica para 2 grupos definidos por frecuencia inicial de convulsiones: <4 y >=4 convulsiones/mes.

RESULTADOS: En la población evaluable (n = 471), la dosis estable promedio de topiramate fue 303 ± 139 mg/d en el grupo con una frecuencia inicial de <4 convulsiones/mes, y 341 ± 153 mg/d en el grupo con una frecuencia inicial de >=4 convulsiones/mes (p = 0.005). Los efectos secundarios mas comunes fueron somnolencia (8.5%), fatiga (7.3%), náusea (5.3%), y mareo (5.0%). Menos de 3% de pacientes reportaron síntomas cognitivas. Cuando se redujo la dosis de agentes anti-epilepticos concomitantes, 14% de los pacientes descontinuaron su tratamiento con topiramate debido a efectos adversos en comparación con 23% cuando la dosis de agentes concomitantes no fue cambiado o cuando fue aumentada.

CONCLUSIONES: Cuando se individualiza la dosis de topiramate según la respuesta clínica, la dosis estable, como terapia adicional, está influida por la frecuencia inicial de convulsiones. La tolerabilidad es major cuando se reduce la dosis de agentes anti-epilepticos concomitantes.
