OBJETIVO: Informar 2 casos del síndrome agudo de lisis de tumor (SALT) asociado al tratamiento de hidroxiurea.

RESUMEN DE CASOS: El primer caso es el de una mujer de 79 años de edad a quien le diagnotican SALT luego de 24 horas de tratamiento con allopurinol, hidroxiurea, e hidratación para una transformación blástica aguda de leucemia crónica linfocítica. El segundo caso es el de un hombre de 76 años de edad medicado de igual manera al presentar leucemia mieloide aguda con una fracción de blastos alta. Sus síntomas se presentaron luego de 12 horas de tratamiento, y las pruebas sanguíneas fueron consistentes con SALT.

DISCUSIÓN: SALT es un desorden metabólico que se presenta debido a destrucción celular de tumores que crecen rápidamente. En dosis estándar, hidroxiurea causa muerte celular en la fase S sin causar lisis celular. Sin embargo, en dosis altas tiene un efecto citolítico posiblemente debido un mecanismo distinto.

CONCLUSIONES: Se presentan 2 pacientes mayores de 75 años con leucemia mieloide aguda que desarrollan SALT luego de terapia con hidroxiurea. Ambos tenían leucemia mieloide con leucocitosis. Los pacientes recibieron dosis altas de hidroxiurea para disminuir las células blancas, y ambos desarrollaron señales y síntomas consistentes con SALT. A pesar de que es raro que la hidroxiurea cause SALT, los pacientes a riesgo deben ser vigilados para esta complicación. Al hacer una evaluación de causalidad utilizando la Escala de Probabilidad de Reacciones Adversas de Naranjo, se determinó que el síndrome de lisis de tumor probablemente fue causado por hidroxiurea.
