OBJETIVO: Revisar la farmacología, virología, farmacocinética, eficacia, seguridad, perfil de resistencia, y uso clínico de tenofovir disoproxil fumarate (Viread, Gilead Sciences, Inc.)

FUENTES DE DATOS: Se llevó a cabo una búsqueda utilizando la base de datos de MEDLINE para el período comprendido entre 1966–agosto de 2002, incorporando los siguientes términos: tenofovir, tenofovir disoproxil fumarate, PMPA, (9-(R)-[2-(phosphonomethoxy)propyl]adenine), y Viread. Se revisaron también extractos de conferencias en las que se trataron asuntos relacionados al Virus de Inmunodeficiencia Humana (HIV).

EXTRACCIÓN DE DATOS: Tenofovir disoproxil fumarate es la pro-droga de un nucleótido que es difosforilado al compuesto activo, difosfato de tenofovir. Este compuesto actúa como inhibidor de la transcriptasa inversa, inhibiendo así la replicación del Virus de Inmunodeficiencia (HIV-1). En estudios clínicos, tenofovir fue efectivo en suprimir el RNA del HIV-1 y en aumentar los contajes de las células CD4. Tenofovir cuenta con una vida media intracelular prolongada, lo que hace posible el que se dosifique 1 vez al día. Como el tenofovir no interacciona con el sistema microsomal del citocromo P450, su potencial de interacciones de drogas es mínimo. Tiene el potencial sin embargo de interaccionar con didanosina. En comparación con otros inhibidores de la transcriptasa inversa, tenofovir podría representar una ventaja en términos de toxicidad y de cumplimiento con la terapia. Estudios actualmente en progreso evalúan la actividad de tenofovir contra el Virus de Hepatitis B.

CONCLUSIONES: Tenofovir ha demostrado tener actividad contra HIV-1 en combinación con otros antirretrovirales. El beneficio de la droga en la intensificación de los regímenes de antirretrovirales altamente activos en pacientes que han sido tratados anteriormente con antirretrovirales ha sido establecido, y los datos preliminares en pacientes que no han sido tratados anteriormente son alentadores.
