OBJETIVO: Informar un caso de rabdomiólisis después del uso concomitante de la simvastatina, un inhibidor de la hidroximetilglutaril coenzima A reductasa (HMG-CoA) de uso común, y el fluconazol, un agente antifúngico del grupo azol.

RESUMEN DEL CASO: Un hombre de 73 años con historia de insuficiencia cardíaca e hiperlipidemia se presentó al hospital 1 semana después de añadir fluconazol a un régimen de medicamentos que incluía 40 mg diarios de simvastatina. El paciente tenía debilidad muscular severa y aumento marcado en la actividad de la creatíncinasa sérica, que se resolvió después de suspender la simvastatina y el fluconazol.

DISCUSIÓN: La rabdomiólisis es un efecto secundario reconocido de los inhibidores de la HMG-CoA reductasa (estatinas), que usualmente ocurre por la interacción con otros medicamentos que inhiben el grupo de isoenzimas del citocromo P450, como los medicamentos antifúngicos del grupo azol. Una evaluación de causa objetiva reveló que probablemente éste era un evento adverso relacionado con el uso de medicamentos. A pesar de que se han descrito interacciones por la combinación de inhibidores de la HMG-CoA reductasa y los antifúngicos del grupo azol, no se había informado un caso de rabdomiólisis probablemente causada por la interacción entre la simvastatina y el fluconazol. Este caso enfatiza la importancia de estar vigilantes ante las interacciones de medicamentos, en particular con medicamentos de prescripción común.
