OBJETIVO: El objetivo de este artículo es desarrollar unas guías para el uso inicial de heparinas de bajo peso molecular (LMWHs) y de otros anticoagulantes en hospitales de cuidado agudo que forman parte de una organización nacional de compras en grupo (GPO).

FUENTES DE DATOS: Se realizó una búsqueda sistemática de la literatura para identificar la evidencia que existe sobre el uso de de varios anticoagulantes en el tratamiento inicial de trombosis de vena profunda y de embolia pulmonar, y en el tratamiento del síndrome coronario agudo. Se utilizó el método de consenso de grupo para desarrollar las guías. Estas guías fueron evaluadas y revisadas por un grupo interno de expertos, así como por un grupo externo de expertos. Las guías finales fueron diseminadas a los miembros del GOP, y se proveyó asistencia en la implantación de las mismas.

RESULTADOS: Las guías desarrolladas son descritas en el artículo. Estas se organizan en base a las opciones terapéuticas recomendadas para cada indicación. Para cada opción, la opinión de consenso se provee en base a la evidenia que existe en la literatura, la comparación de costo, conveniencia e información adicional sobre dosificación. Las guías fueron diseminadas junto con material de referencia a los hospitales miembros del GPO, y se coordinaron teleconferencias para facilitar la implantación a nivel local. Las guías fueron implementadas inicialmente en 18 hospitales a nivel nacional.

RESUMEN: El proceso mediante el cual estas guías se desarrollaron, al igual que las guías puede ser de utilidad para hospitales y sistemas de salud que contemplan o han llevado a cabo un esfuerzo similar con esta clase de fármacos.
