CARDIOLOGY

TRASFONDO: Argatroban es un inhibidor directo de trombina usado para tratar trombocitopenia inducido por heparina (HIT, por sus siglas en inglés). Argatroban se depura primordialmente por mecanismos hepáticos con sólo pequeñas cantidades depuradas por los riñones como fármaco sin cambiar.

OBJETIVO: Evaluar los efectos de la función renal en la dosis de argatroban y en el tiempo parcial de tromboplastina activada (aPTT, por sus siglas en inglés).

MÉTODOS: Pacientes tratados con argatroban fueron identificados y escogidos prospectivamente. Pacientes con disfunción hepática fueron excluidos del análisis. Se repasaron expedientes y resultados de laboratorios diariamente hasta alcanzar un aPTT terapéutico o que argatroban fuera descontinuado. Los datos recolectados incluyen edad, peso, género, diagnóstico para admisión, historial médico pasado, indicaciones para anticoagulación, indicación para argatroban, dosis inicial, aPTT deseado, instrucciones para titulación, pruebas de función hepática, creatinina sérica (Scr, por sus siglas en inglés), nitrógeno de urea en sangre, y depuración estimada de creatinina (Clcr, por sus siglas en inglés).

RESULTADOS: Un total de 66 pacientes fueron evaluados y 44 reunieron los criterios de inclusión. La Scr de base estaba elevada a 1.5 mg/dL (0.9, 2.3) (25ta, 75ta mediana) con una Clcr estimada de 40 mL/min por 1.73 m2 (26, 74). La mediana de la dosis de argatroban para alcanzar el rango terapéutico predefinido fue 1 µg/kg/min (0.68, 2) con un aPTT correspondiente de 60 segundos (54, 66). Después de un análisis de una variable, Clcr predijo significativamente la dosis terapéutica (coeficiente b ± SE = 0.019 ± 0.004, r2 = 0.35, p < 0.001). Covariables que predijeron la dosis fueron la presencia de HIT (coeficiente b ± SE = –0.61 ± 0.3, p = 0.045), historial de MI (coeficiente b ± SE = –0.74 ± 0.3, p = 0.02), y una indicación para anticoagulación de DVT/PE (coeficiente b ± SE = 0.69 ± 0.3, p = 0.03).

CONCLUSIONES: La Clcr estimada predijo significativamente la dosis de argatroban necesaria para alcanzar un aPTT terapéutico.
