OBJETIVO: Comunicar un caso de erupción psoriasiforme inducida por infliximab.

RESUMEN DEL CASO: Una mujer de 46 años con fístula enterocutánea secundaria a enfermedad de Crohn desarrolló placas eritematosas, descamativas y pruriginosas en los codos, las rodillas, las manos y los glúteos tras recibir la segunda y tercera dosis de un tratamiento con infliximab intravenoso. El estudio histopatológico mostró un patrón liquenoide. No aparecieron nuevas lesiones tras suspender el tratamiento con infliximab.

DISCUSIÓN: Se han descrito diversos tipos de reacciones cutáneas secundarias al infliximab, un anticuerpo monoclonal anti-TNF-alfa. No se ha descrito con anterioridad una dermatitis psoriasiforme como reacción cutánea por infliximab, pero existen múltiples comunicaciones sobre dermatitis psoriasiformes secundarias a otros fármacos. Una evaluación objetiva de la causalidad determinó este efecto adverso como probable.

CONCLUSIONES: Se describe un caso manifestado clínicamente como una erupción psoriasiforme y que mostraba una imagen histopatológica de dermatitis liquenoide. Este patrón clínico-patológico disociado de reacción cutánea no ha sido referido con anterioridad como un efecto secundario del tratamiento con infliximab.
